If I recall correctly, the Dodongo's Cavern of which you speak is the place near the entrance to Snowfall where you can bomb the floor and find a grotto. Inside is a bunch of Dodongos.
That place is never called "Dodongo's Cavern".
The mention of Dodongo's Cavern in MM was only an allusion to OoT.

1)FSA comes right before the IW. It just seems so right.
2)Termina and Hyrule are closer related than many people think. I bet Terminians can go to Hyrule and Hylians can go to Termina. I mean, who built the Forest Temple? The Kokiri? The Hylians probably know something about the Lost Woods, just not about the Kokiri. Either that or the Terminians built it. But anyway, I just think Termina is sorta another land. Maybe that tunnel in the Lost Woods is just the only way to get there and after walking a long way through the tunnel one would find themselves in Termina.

2) No, the Sages built the original Hylian Temples, obviously. All the temples are sanctuaries for a specific Sage, which implies each Sage created their own temple. The Terminians? They're not involved at all. The Temples were built by the Sages and united to act as a complex, elaborate seal on the Sacred Realm.
3) Termina is an alternate dimension. It's fact. The tunnel Link traveled through is an allusion to Alice in Wonderland, just like the entire MM game.

I think that the ancient Hylians probably built the Forest Temple, but I doubt that they ever made it into Termina. I think that the only reason that Link was able to travel to Termina was because some higher power sent him there to save it.

Maybe I'm reading too much into it, but that amibiguity in how they define it makes it sound, to me at least, like it may not neccessarily be an alternate dimension in the clasic sense.
Naturally, without any evidence to support anything specific, I won't go into any fan-ficcy spooging.

It's slightly ambigious but not TOO ambigious. I think it's basically saying it is a parellel world (and thus a sort of 'alternate dimension') but it's clearly not completely parellel because there are a series of differences between Termina and Hyrule.
So, well, you're right, it's not an alternate dimension in the classic sense. It's a different world that, as the manual says, is similar yet different to Hyrule.
Nothing more than that, we don't need to think about it too much. 'Tis magic.
